PA Bureau of Family Health-Adolescent Health Survey

The Bureau of Family Health is in the process of completing a five year needs and capacity assessment. As part of this process the Bureau has developed an adolescent health web survey. The information collected from this survey will help the Bureau determine what the needs are of adolescents in the Commonwealth and hopefully direct funds to address those needs.
