Below are forms and documents used by the Main Office at Northern Lehigh Middle School.
When a student is absent from school, he/she has THREE (3) school days to submit a valid written excuse. Please feel free to use the attached forms. Refer to student/parent handbook for conditions/situations that constitute reasonable cause for absence.
NOTE: If a student is mandated, then a doctor’s excuse is required.
