The only hard copy form that must be submitted is Section 6 of the PIAA Pre-Participation Physical form. This is the section that is completed by the Certified Medical Examiner and must take place June 1st or after for next school year. Any physical done before June 1st is not valid. This hard copy must be submitted to the Northern Lehigh Athletic Trainers.
