These forms are distributed to all students on the first day of school and should be completed and returned within a week. Students moving into the district during the school year will receive these forms upon enrollment, and should be returned within a week.

If you are a new (or renewing) volunteer to Slatington Elementary School, you must have the following forms on file in our office. These forms are needed for you to chaperone field trips and participate in any capacity that involves direct contact with students. Thank you for your interest in volunteering at Slatington Elementary School.

Clearances:

PA State Criminal Background Check (Volunteer Form) - Indicate "VOLUNTEER" to have the fees waived for this form.

Child Abuse Clearance (Must create account and clearance form online) - Indicate "VOLUNTEER" to have the fees waived for this form.

FBI Clearance (Only needed if you moved to PA within the past 10 years)

Affidavit of Residency in lieu of FBI Fingerprint Background Check (Can be signed in front of office personnel to waive fingerprinting requirement if have lived in PA for the past ten years.)

Educational Field Trip Form - Please complete this form prior to any educational field trips you will be taking with your child during the school year. Educational field trips must be approved by the building principal and superintendent for your child's absence to be considered legal. It is important that you complete this form for approval prior to your trip. Additionally, please understand your child will be held responsible for making up any missed work or exams.

Birthday Basket Form - This service is provided by our Food Services Department. If you would like to purchase a birthday basket for your child, please print out this form, complete the information, include cash or check, and return to school.  Birthday basket options include your choice of: a prize basket for your child to share with classmates, an ice cream treat for your child and classmates, or a snack bag for your child and classmates.

CHIP - Children's Health Insurance Program - Pennsylvania Act 84 of 2015 requires that public and nonpublic schools receive electronic notice with general information about the Children’s Health Insurance Program (CHIP) on an annual basis no later than August 15. The attached flyer was created and should be shared with every parent or guardian of every student enrolled in the local education agency (LEA) during the  school year.
