This form must be completed by a representative of any organization that wishes to use Northern Lehigh School District facilities (i.e. auditorium, cafeteria, classroom, kitchen, gymnasium, refreshment stand, etc.) for their own organization's events or activities. Upon completion of form, please email to Athletic Director, bgeist@nlsd.org, and Director of Operations, cmann@nlsd.org, for approval.
